If you are a postmenopausal woman between 50 and 70 years of age, answer the questions below to the best of your ability. These answers may help your doctor determine if there is a need for further testing for osteoporosis.

What does your SCORE® mean?

If you are between the age of 50 and 59 and your final SCORE® is 6 or greater, you should be evaluated further for osteoporosis.

If you are between the age of 60 and 70 and your final SCORE® is 8 or greater, you should be evaluated further for osteoporosis.

While this questionnaire is not 100% predictive of osteoporosis, it may strongly indicate the need for appropriate evaluation.
